2019 Community Health Improvement Plan
The 2019 Community Health Improvement Plan, developed with Clear Impact Scorecard, was written as a result of the 2018 Community Health Assessment. To view a PDF, please click here. Data reviewed during the CHA process led to choosing the top three health priorities: mental health and substance use, perinatal and early child health, and chronic disease prevention. The CHIP contains action plans addressing each of these priorities.
